(A Patient’s Experience, And a Cybersecurity Professional’s Critique)

The solution involves making technology work for clinicians rather than against them, while building genuine transparency and accountability. Staff will always prioritise patient care over cybersecurity compliance, and our solutions must work with this reality.

Introduction

The NHS is the UK’s lifeline. Millions depend on it every single day, from routine check-ups to life-saving interventions. The clinicians and frontline teams often perform miracles despite constant pressures and dwindling budgets. But beneath the surface, the NHS has a growing problem that doesn’t make the evening news until it explodes: its digital security and data handling practices are often outdated, fragmented, and vulnerable.

This isn’t about taking cheap shots at a public institution. It’s about recognising that the NHS has become a prime target for cybercriminals, ransomware gangs, and data thieves, not because staff don’t care, but because the systems, culture, and priorities simply don’t match the realities of the digital age.

Numerous national audits and studies continue to highlight that legacy IT and varying security standards across trusts make the NHS an attractive target for cyberattacks, causing widespread staff frustration with poor digital tools.

Cybersecurity concerns end up routed through clinical managers rather than IT or security teams, because that’s how their complaints process is designed, and that process itself is failing. These aren’t just harmless quirks or one-off slip-ups; they’re systemic cracks, leaving the NHS exposed to both external threats and self-inflicted damage.

The challenges I share are echoed in widespread staff surveys and patient reports, which routinely highlight IT frustrations, pressure to find workarounds, and a lack of confidence in digital safeguards.

It’s important to acknowledge: NHS Digital, NHSX, and the National Cyber Security Centre (NCSC) do have initiatives to improve standards. But effects “on the ground” remain patchy, and many issues persist.

Rogue Networks and Shadow IT

Take this example: one evening in hospital, my phone picked up a new Wi-Fi network named “IWillHackYou”. For a split second, I assumed we were under attack. It turned out to be an agency member of staff using their phone as a hotspot, so that clinicians could get online during their shift.

Convenience trumps policy, and soon this ad-hoc network became the default for staff notes, app chats, even clinical data passed through WhatsApp. It’s easy to understand, hospital wireless can be painfully slow or restrictive, and, when patient care is the priority, people find workarounds. But every time they connect to these unofficial hotspots, they’re bypassing all the hospital’s controls. There’s no encryption guarantee, no oversight, and no assurance your information stays within NHS control. Agency staff, by the way, move on quickly, and with a network name like “IWillHackYou”, it’s not clear if it’s a joke or a warning.

I did formally report the incident both at the time and formally later, but the trust’s complaints team treated it as business as usual. This isn’t a policy quirk, it’s a security hole, and I’ve seen it play out repeatedly.

Nationally, shadow IT is identified as a leading source of vulnerability, with NHS Digital and NCSC highlighting the need for clear guidance and real enforcement, not just documentation of ideal policy.

Insider Risks: Beyond Accidental Breaches

While staff workarounds often stem from good intentions, the risk of insider threats, whether deliberate or accidental, cannot be understated. During my three-month stay on an NHS ward recovering from sepsis, I witnessed firsthand how system frustrations create security vulnerabilities that go far beyond policy breaches.

I’ve seen junior doctors photographing clinical records on their personal devices rather than waiting for slow systems to load or dealing with unavailable terminals when pressed for time. When patient care is urgent, convenience trumps security protocols. But those photos, containing sensitive patient data, now live on personal devices with unknown cloud sync settings and backup arrangements.

The authentication problems I mentioned earlier create their own insider risks. Ward pharmacists, nurses, and ward sisters routinely share credentials or use a single staff member’s login for an entire shift because the sign-in process is too lengthy. Many nurses told me they simply don’t log into their NHS accounts to read important memos because the system is too clunky and time-consuming, they just want to get on with caring for patients.

When I reported these digital security concerns, the same trust where I’d flagged the “IWillHackYou” network and the Remarkable2 incident, I was met with “What are we meant to do?” from clinical staff and effective non-responses from management. The reality is that digital security concerns are routinely handed to clinical complaints managers who lack the technical knowledge to understand or address them properly.

Cases of staff accessing records without clinical need, or data leaking through these informal workarounds, should lead to proper investigation and system improvements. Instead, the NHS appears to treat these as inevitable byproducts of underfunded IT rather than serious security incidents requiring both technical and cultural responses.

Unapproved Devices and Cloud Sync

During one formal complaints meeting, I noticed the Facilities Manager wasn’t using a laptop or a standard NHS-issued device to take notes. Instead, they were using their personal Remarkable2 tablet. On the surface, it looks harmless, just a sleek, paper-like digital notebook. But these devices usually sync automatically to personal cloud services, most commonly Google Drive.

In this case, sensitive clinical data was being discussed, and the Facilities Manager was actively making notes on a personal, non-corporate device. That means patient details, contract notes, and clinical references were likely stored outside the NHS’s secured environment, landing in a private account beyond organisational control. As far as I’m aware, devices like the Remarkable2 aren’t on any NHS-approved hardware list, precisely because of these uncontrolled sync behaviours.

For clarity: NHS Digital requires corporate mobile devices to be centrally managed and only allows use of NHS-accredited applications/services for storing or syncing patient-identifiable data. Personal cloud solutions are explicitly not permitted for clinical records, and unapproved devices are a clear compliance failure under NHS Digital’s technical and GDPR requirements.

When I raised it informally, I was met with blank stares, as though the concern didn’t register because the device wasn’t a traditional laptop. There seemed to be little awareness (or interest) that sensitive data could be flowing straight into unmanaged, non-compliant cloud storage. Whether through oversight or indifference, this kind of shadow IT puts both commercial and clinical information at unnecessary risk.

Digital Communication and PII Exposure

Across multiple NHS trusts, I’ve seen sensitive documents, appointment letters, test results, even financial details, sent out by email with no encryption, often to personal accounts. When questioned, staff sometimes point to “TLS encryption” as their safety net, but TLS only protects the link between mail servers.

It’s like writing your most personal details on a blank postcard and trusting it to the postal system, hoping no one along the way reads, photocopies, or shares it. And when that postcard lands in the hands of a nosy postman, nothing is private anymore.

This confusion often comes from mixing up two different things: standard TLS between email “hops”, and the type of certificate-pinned mailserver connections used between legal entities for contracts, where encryption is effectively end-to-end until the message reaches the recipient organisation’s secure mail server.

NHSmail’s ‘accredited’ encrypted channel with other NHS and government partners does offer greater protection, but most outbound communications that leave the NHS ecosystem, especially those sent to patients’ private accounts, are not protected to the same standard.

It isn’t just outbound communication, either. NHS PALS teams, consultants’ personal assistants, and complaints teams frequently ask patients to confirm their name, address, date of birth, NHS or hospital number, and other identifiers via cleartext email. I’m currently challenging this practice with the ICO, as trusts appear to think it’s acceptable so long as they bury a disclaimer in tiny print at the bottom of the request email, a disclaimer most people won’t read or understand.

Missed opportunities for better staff guidance and robust privacy-preserving alternatives, like secure patient portals, or touchpads/kiosks at reception to enter details privately, should be addressed nationally rather than left to local discretion.

Reception and Physical Security Risks

The risks aren’t confined to digital communications. In nearly every department reception, patients are routinely asked to provide full personal details aloud:

  • Name
  • Date of birth
  • Address
  • GP details
  • Next of kin
  • Last four digits of a phone number
  • and more…

Anyone in the waiting area could overhear and jot down enough information to piece together a profile. For those uncomfortable with saying it out loud, a paper slip might be offered, but these are often scrunched up and tossed in ordinary bins instead of going into secure disposal. I’ve even seen patient notes shredded with a basic domestic strip shredder at an NHS dentist, leaving wide, reconstructible strips of sensitive information.

It’s inconsistent, outdated, and careless. Not only does it breach the spirit of GDPR and NHS data protection standards, but it also leaves patients exposed to identity theft or fraud before they’ve even seen a clinician.

Sluggish Systems and Unsafe Shortcuts

Spend any time on an NHS ward and you’ll quickly see the frustrations staff face with their IT systems. Logins can take over a minute, software lags at the worst moments, and hardware feels like it belongs in a museum rather than a modern hospital. Many staff use ICC (smart) cards to log into terminals, but because the process is slow, they often leave devices logged in and unattended. Others, when pressed for time, simply share credentials so they can get on with caring for patients instead of fighting a login screen.

Training on new software updates is also slow and inconsistent, largely due to staff shortages, increased workloads, and the sheer reality that emergencies and direct patient care always take priority. Staff frequently complain about poor software interfaces, sluggish behaviour, clunky updates, and complicated user interfaces designed by people who clearly haven’t spent a day on the floor of a ward. Confusing menus and awkward pathways only make things worse, especially for those staff who aren’t particularly IT literate and would rather avoid the tech entirely.

But those same staff who struggle with the tech? They’re the ones you absolutely want by your side when your heart stops and you’re being jump-started with a defibrillator. I should know, I’ve technically died twice so far in my lifetime, and I can tell you I was far more grateful for their clinical skill with a defibrillator than their computer literacy.

This widespread user frustration and forced workaround behaviour is well documented in surveys and case studies, reinforcing the need for systems designed around users, not just compliance and policy. Investing in user-friendly interfaces, rapid login solutions, and workflow-driven design could be transformative.

The root of the problem isn’t laziness or ignorance; it’s infrastructure and systems that don’t support the realities of clinical work. Sluggish systems, confusing designs, and lack of time for proper training force staff into risky shortcuts, undermining the very policies meant to protect patients and data. Until the NHS fixes its foundational IT performance and usability issues, it’s unrealistic to expect perfect compliance from staff whose first priority will always be patient care.

Disjointed Systems – and CDs in Taxis

One of the most telling examples of how fragmented NHS systems can be happened during my late wife’s care. She had an MRI scan at Trust A, ordered by Trust B. Rather than securely transferring the data electronically, the scan was burned to a CD and sent over 60 miles by taxi so Trust B could load it into their system. When I asked why they weren’t using secure file transfer methods, SFTP, Connect Direct Secure+, or any modern solution, I was met with blank stares. It was as though the concept didn’t even exist in their processes.

The absurdity doesn’t stop there. Recently, I was being treated by Trust A, while Trust C operated from a building directly opposite. Yet, despite being within shouting distance, neither trust could share my records because they were running entirely separate, incompatible systems. The result? Repeated questions, duplicated tests, and wasted time for both patients and staff.

This fragmentation isn’t just between trusts, sometimes it’s within the same trust. Just this week, during a routine Pre-Op Clinic assessment, the nurse queried why I hadn’t listed certain medications on the digital form. For context, patients are sent a link by SMS asking them to review and update their pre-surgical information. Since I’d had surgery earlier this year, I assumed the form would contain up-to-date data. But instead, they were referencing my pre-op assessment record from 2022, not the version submitted in 2025.

It turns out each clinical directorate within the same NHS trust creates a separate patient record, even though they all use the same software platform. So your cardiology notes, orthopaedic data, and surgical history can live in parallel silos, duplicating information across departments with no unified source of truth.

Not only does this create multiple versions of the truth, it introduces countless opportunities for human error. When I raised the issue, the staff were sympathetic but blunt: they were overwhelmed, under-resourced, and had no faith that flagging the problem would change anything.

Instead of acknowledging the system failure, I was subtly reprimanded for the inaccuracy, despite the fact that they had pulled the wrong record. When outdated data is being served by design, and patients are blamed for it, we have a cultural and technical failure. It’s unreasonable to expect patients to manage the integrity of their record across disconnected silos they can’t even see.

This is emblematic of a deeper issue: decision-makers in the C-Suite may believe they’re solving problems by investing in digital systems, but when those systems are siloed, misused, or ignored by overburdened frontline staff, the gains are illusory. The ground truth isn’t in the tech stack or the PR statements, it’s in the waiting rooms, clinics, and corridors where mismatched records and duplicated questions are a daily frustration.

While some trusts are reportedly piloting better solutions, Leeds Teaching Hospitals NHS Trust with a FHIR-based interoperability platform, Guy’s and St Thomas’ with patient-centred portal design, these remain exceptions rather than the norm. Across my own experiences with multiple trusts, I haven’t encountered systems that genuinely work well for patients or staff. The reality is that digital transformation announcements rarely translate into functional improvements at the point of care.

Transparency? Only After You Fight for It

For all the talk of digital transformation, there’s still a culture of secrecy around patient information in the NHS. Patients are routinely told what their doctors think they need to know, but rarely shown the raw data, whether it’s X-rays, MRI scans, or contemporaneous clinical notes. In many cases, consultants will summarise findings but avoid pulling up the images or records on screen, leaving you to take their word for it.

In my late wife’s case, we only discovered how inaccurate and misleading some of the contemporaneous notes were after submitting a Data Subject Access Request (DSAR). These notes didn’t reflect what was actually said or agreed during appointments, and we were never given the chance to correct or even challenge them at the time.

Even filing a DSAR is an uphill battle. I’ve had trusts refuse to process my requests unless I used their dedicated online portal, despite GDPR making it clear you can raise such requests by email, in writing, or verbally. It took ICO involvement to get my requests honoured, and even then, some data, like CCTV footage, was missing. Promises were made about staff training and process changes, but when I tested it again months later, nothing had improved. Two years on, the same barriers and excuses remain.

Even the official NHS App reflects this disconnect. For all the money spent developing it, it feels like a glorified wrapper around a webpage, sparse, clunky, and often little more than a summary of a summary. Compare it to apps like Airmid UK, which connects to the same SystmOne backend but offers GP notes and far richer detail, and the contrast is staggering.

Patient access to full records, the right to correct errors, and informed consent are all guaranteed by law, but in practice these rights are inconsistently honoured. Further improvement here is essential, not only for transparency, but to empower patients and ensure accuracy of their healthcare record.

Patients shouldn’t have to fight to see what’s written about them, nor should they need to become data protection experts just to hold the system accountable. Yet in my experience, that’s exactly what it takes.

True patient empowerment means more than just access to records.

Patients should also be involved as co-designers of the very systems they’ll use or trust. Including patient reps on cybersecurity working groups and in user-testing would spot usability and privacy issues before anything goes live, making cyber safety a shared responsibility, not just an expert item.

When the Cracks Turn into Catastrophes

When these systemic vulnerabilities, from shadow IT to data silos to transparency failures, aren’t addressed, the consequences extend far beyond individual frustrations. The cracks I’ve witnessed firsthand aren’t theoretical; they’re the same weaknesses exploited in real-world cyber disasters that have severely affected NHS services and patients:

  1. WannaCry (May 2017): The ransomware attack disabled systems across 47 NHS organisations, cancelling around 6,900 appointments initially, and reaching an estimated 19,000 total cancellations, with combined losses around £92 million in IT restoration and lost care capacity.
  2. Synnovis Ransomware Attack (June 2024): Synnovis, a key pathology provider for London trusts, was hit by the Qilin ransomware gang, resulting in 400 GB of data breached and estimated disruption costs of £32.7 million (far exceeding prior profits).
  3. NHS Dumfries & Galloway (March 2024): A breach exposed approximately 3 TB of patient and staff data, affecting around 150,000 individuals, with data ultimately published online.
  4. HCRG Care Group (Feb 2025): The Medusa ransomware attack exfiltrated over 50 TB of sensitive data and led to a $2m ransom demand, a critical supplier breach with direct NHS impact.

Repeated incidents underscore that technical advice, policy, and even established security teams alone are not enough: post-incident learning, well-rehearsed playbooks, and real investment in foundational IT: patching, backups, monitoring, communication are crucial to breaking the cycle of breach and blame.

Networked Devices & IoT: The Weakest Link?

Another emerging concern is the proliferation of networked medical devices, such as infusion pumps, vital sign monitors, and legacy imaging equipment, many of which run outdated software or rely on insecure network protocols. Each connected device represents a potential entry and pivot point for attackers.

During my three-month ward stay, most of the equipment I encountered was outdated, a sad reminder of the chronic underfunding that affects not just staffing, but the very tools meant to monitor and treat patients. When medical devices are running obsolete operating systems or haven’t been updated in years, they become easy targets for cybercriminals looking for network access points.

The challenge isn’t just technical, it’s financial and operational. Replacing or updating medical equipment requires significant capital investment that many trusts simply can’t afford. Meanwhile, these devices remain connected to the same networks that handle patient records and clinical systems. A practical solution would be to segregate medical devices onto separate networks, isolated from systems containing patient data, though it’s unclear how many trusts have implemented such network segmentation.

NHS trusts and suppliers must ensure that every piece of internet-exposed or wireless equipment is secured, monitored, and regularly updated, lest a vulnerable medical device become the vector for a breach or patient safety incident.

The Threat Horizon: AI and Advanced Attacks

The NHS also faces an evolving threat landscape shaped by advances in artificial intelligence and automation. Cybercriminals are deploying AI-driven phishing campaigns, deepfake voice impersonations, and automated reconnaissance tools that can evade traditional defences and exploit human vulnerabilities at scale.

This is particularly concerning given that most NHS staff and patients aren’t IT security experts. The sophisticated social engineering techniques that AI enables, convincing fake appointment confirmations, bogus health surveys, or impersonated clinical communications, could easily fool busy healthcare workers focused on patient care rather than cybersecurity threats.

These technologies raise the bar for detection and response, making it more urgent than ever for the NHS to invest in adaptive security measures and continuous staff awareness programmes. The sophistication and volume of such attacks are only expected to grow.

Ransomware Stance vs. Reality

The official stance from the UK government and the National Cyber Security Centre (NCSC) is firm: do not pay ransoms. As of July 2025, the government has announced plans to ban public sector organisations, including the NHS, from paying ransom demands outright, alongside introducing mandatory incident reporting.

The message is clear: paying only fuels criminal enterprises, offers no guarantee of data restoration, and organisations must focus on prevention and recovery instead.

The reality on the ground, however, is far more complex. In sectors where downtime can cost lives, like hospitals or critical suppliers, organisations often quietly engage specialist brokers to negotiate with ransomware gangs. These intermediaries provide a layer of separation, allowing ransom deals to happen without directly breaching policy, because the alternative can be catastrophic disruption to essential services.

Multiple case studies and investigative reports confirm this gulf between official policy and operational responses, further evidence that better prevention, resilience, and recovery capacity must be funded before a real ban can work.

This disconnect between policy and practice exemplifies the broader challenge facing NHS cybersecurity: well-intentioned directives from the top that don’t account for operational realities on the ground.

Solutions and Recommendations

Pointing out the NHS’s cybersecurity shortcomings is only half the story. The real question is: how do we fix this without punishing clinicians or bankrupting the system? Meaningful change needs to focus on practical, patient-safe solutions that tackle the causes, not just the symptoms.

The government’s Cyber Security Strategy for Health and Adult Social Care to 2030 sets out five key pillars that provide a solid foundation:

  • Focusing on greatest risks
  • Defending as one
  • Improving people and culture
  • Building secure for the future
  • Exemplary response and recovery.

These pillars align well with many of the issues I’ve observed, but implementation at ground level needs to address the practical realities that frontline staff face daily.

Building on this national framework, here are recommendations prioritised by implementation timeline, including gaps that need addressing:

Quick Wins

  • Replace vague disclaimers with real guidance on email security, encryption, and PII handling – addressing the confusion around TLS vs end-to-end encryption I’ve witnessed
  • Provide touchpads or kiosks at receptions so patients can enter details privately, without broadcasting them aloud in waiting areas
  • Raise awareness of patients’ rights under GDPR and ensure NHS processes clearly communicate those rights in practice, not just policy

Short-Term

  • Mandate Cyber Essentials Plus for every trust as minimum baseline – aligning with the strategy’s CAF implementation through DSPT by 2025
  • Require offline, regularly tested backups for every trust with documented recovery procedures
  • Hold suppliers to minimum security accreditations (Cyber Essentials or ISO 27001) – supporting the strategy’s supplier engagement and criticality mapping by 2024
  • Foster a “just culture” for cyber incident reporting where staff feel protected to flag concerns without disproportionate blame

Medium-Term

  • Implement single sign-on or virtual desktop solutions to eliminate the shared credentials and logged-in terminals I observed during my ward stay
  • Adopt secure, modern transfer standards (SFTP, Connect Direct Secure+, FHIR APIs) to eliminate the “CDs in taxis” problem
  • Establish public “cyber resilience report cards” for each trust, published annually with metrics like patch compliance, MFA rollout, and incident response times
  • Improve transparency around external consultancy effectiveness – publicly report what major consultancies deliver against investment, including measurable improvements in patch rates, MFA rollout, SOC coverage, and incident response times
  • Balance external expertise with in-house capability building to reduce long-term dependency on contractors whilst leveraging specialist knowledge where appropriate

Long-Term

  • Enhance the centralised NHS Security Operations Centre (already planned for 2024 in the strategy) and establish regional support centres
  • Upgrade outdated endpoints and network infrastructure that force staff into risky shortcuts
  • Introduce federated data model for secure cross-trust queries, ending the siloed records problem
  • Build strong in-house cybersecurity teams whilst making appropriate use of external expertise, reducing over-dependency on contractors for core security functions

Addressing the Gaps

Several critical areas need attention beyond the current strategy:

Patient-Facing Security: The strategy focuses on organisational cyber resilience but overlooks patient digital literacy. NHS should invest in public campaigns about cyber risks, phishing tactics, and how to safely interact with digital NHS services.

Physical Security Integration: The strategy emphasises digital threats but misses basic physical security failures like patients stating PII aloud in waiting areas and poor document disposal practices.

Ground-Level Implementation: While the strategy sets excellent high-level direction, it needs clearer guidance on addressing the day-to-day workarounds that create vulnerabilities – from shadow IT to shared credentials driven by system frustrations.
